Simply name it, set it to open as an app, then click "add to home screen".

To see the setup menu, click on the icon at the top left of your dashboard.
Click on the settings button. You'll then be taken to your rewards setup screen where you can customise rewards.
Set your reward criteria, choose your card colour, upload a logo, save your settings... Your set!
